Michael Davidsaver 97407cd11e client: fix Op early destruction 
Errors resulting in early Op destruction would erroneously
be escalated to "Not in running evbase worker" exceptions.
Something has already gone wrong, but this would mask the
root cause.

Mike Smith 4af3028930 OperationBase::chan is nullptr until Channel is created, check before getting name 
have name() return the channel name from OperationBase, not OperationBase::chan::name
