Most errors found with:

c++20: Do not use apostrophe (e.g. can't) in unquoted #error message

3c99391d93 Added SPDX License ID to all EPICS-original source files 
In some cases the license-identification header was missing,
so I added that as well. Replaced the remaining headers that
specifically identified "Versions 3.13.7 and higher".

Makefiles and the build system were deliberately excluded.

1199fabe90 Fix strange compiler errors with gcc 2.96: 
using <namespace::function>; seems not to work but
using namespace <namespace>; works
Also 'add' only works with explicit namespace. Why?
