libCom: joinable threads (posix only)
Add epicsThreadJoin() and epicsThreadOpts::joinable.
For compatibility, default threads aren't joinable
by default.
Currently only POSIX can join.

Variant of epicsThreadCreate() which accepts optional
arguments via a structure to allow for compatible
addition of new arguments.
